Scott McTominay will step away from Napoli duty in the coming days to undergo heart surgery after the club revealed he has developed a “mild benign arrhythmia”.
The 29-year-old midfielder, a cornerstone of Napoli’s recent success since his £25 million move from Manchester United in 2024, will have a corrective procedure in Italy and is expected to be sidelined until after the international break.
Napoli move quickly to calm fears
Napoli announced the news on Tuesday, stressing both the nature of the problem and the expected timeline for his return. The condition, doctors have indicated, is relatively common among elite athletes and is not expected to threaten his long-term career.
In a club statement, Napoli said: “Following the onset of a mild benign arrhythmia, Scott McTominay will undergo corrective surgery using a cardiac arrhythmia ablation (PFA) in the coming days. He will be available again after the international break.”
The diagnosis arrives just days after McTominay’s last outing, a shock home defeat to Como at the weekend. That appearance took his tally for Napoli to 82 games and 27 goals, a remarkable return for a player who has reinvented himself as a driving, goal-scoring force from midfield in Serie A.
For Scotland, he remains just as central: 73 caps, 15 goals, and an ever-present influence.
Allegri’s early reign takes another hit
The timing could hardly be worse for Massimiliano Allegri. Already under scrutiny during a choppy start to his tenure, he must now navigate a crucial run without the man who has become his midfield anchor.
Before the heart issue was made public, Allegri had hinted at fitness concerns following the defeat to Como, though at that stage the focus was on something far more mundane.
“McTominay had some problems with blisters, so he’s been training on a limited basis. Hopefully, we can get him into the best possible condition as soon as possible,” the Napoli manager said.
Instead of simply managing minor niggles, Allegri now finds himself reshuffling a side that has already undergone heavy surgery of a different kind.

Napoli insist McTominay’s absence will be temporary. If his recovery follows the expected path, he should be back after the break, with the home match against Frosinone on October 10 pencilled in as a potential return date.
By then, Napoli’s season could look very different — for better or for worse.
